EFA Business Agreement
I acknowledge that the facts set forth in this application are true and complete and understand that if accepted into the EFA program, any false statement on this application shall be considered sufficient cause for immediate removal.
For the purposes of determining the suitability and qualifications as a business participant, I authorize this company or its security agent, to contact orally or in writing any third parties to obtain information which this company or its security agent deems necessary and appropriate in verifying my application and qualifications as a business.
I specifically authorize this company or its security agent to:
Obtain information from any federal, state or local law enforcement agency, including records of any Criminal History Record information and Motor Vehicle Records.
Obtain information concerning any retail credit from credit reporting agencies and financial institutions.
Obtain available records from state worker’s compensation boards, insurance carriers or other agencies as a means of verifying information provided on this application.
I also request that these persons and institutions cooperate with any agents with respect to these requests for background information. I hereby waive any rights or claims I might have against its agents, including any other person providing the requested information with regard to the acquisition, use, retention or disclosure of any such information.
My business agrees to maintain the proper license, bond or insurance as required by the standards of its individual industry.
I agree to implement the program into my business in the following manner:
Use this program to promote only goods and services that are consumer oriented in nature.
Define my business accurately within the categories listed in my profile as pertaining to my product or service.
Develop an internal tracking process applicable to my business.
Donations and marketing fees will be paid promptly and will be reported accurately in conjunction with accrued revenue via the electronic reporting structure contained within this website per the schedule established in the General Guidelines for Business.
